                                      Order Form
     This is the easiest and most accurate way to place an order. (If you would rather
place your order over the telephone, you may call 1-800-527-7135. We are open
Monday thru Friday from 8:30 am to 5:00 pm Central time). Using this form will ensure
accurate spelling of your shipping address and email.   We will use your email to send a
package tracking confirmation. After you submit this order form, we will make sure the
items are in stock and available to be shipped. We will then call you to confirm the order.
If you submit the form after hours, we will contact you the next business day. We do not
ask for your credit card information on this form. We will ask for it when we speak with
you by telephone to confirm the order.   If you prefer that we not contact you by phone or
if you just prefer not to give out your credit card information over the telephone, we will
be happy to email you a Paypal invoice with a link that allows you to pay with your credit
card using the secure Paypal system. You do not have to be a Paypal member to use
this payment method.   We will ask for your preference below.

If you are ordering dresses or accessories that need to be shipped to different
locations, you will need to fill this form out and resubmit it for each item or group of items
that have a different "ship to" address.

Ship by: UPS Ground, UPS "2 Day" Air, UPS "Next Day" Air, Priority MAIL, or No preference-Send cheapest
please check the shipping prices on the ordering information page. Air freight is more expensive.
